BayCare Launches New Mobile Application for Patient Discharge Instructions

Clearwater, Fla., – To make it easier for hospital patients to transition back to their home environment, BayCare Health System is launching an application that allows people to receive discharge instructions on their mobile devices.

Utilizing the cell phone number provided during patient registration, this new application securely delivers and confirms receipt of electronic discharge instructions sopatients can see things like prescribed medications, diet, activity and recommended follow-up appointments. Meanwhile, BayCare providers can rest assured that this information can go anywhere their patients do.

MU2 Thresholds Exceeded
The innovative application, powered by CareNotify™, has been available for several weeks at St. Joseph’s Hospital, St. Joseph’s Children’s Hospital, and St. Joseph’s Women’s Hospital in Tampa. Early indications show that more than 25% of patients are taking advantage of the new mobile technology.

Operational Savings
Today, the application was introduced tofive additional BayCare hospitals: St. Joseph’s Hospital-South in Riverview, St. Joseph’s Hospital-North in Lutz, South Florida Baptist Hospital in Plant City, Winter Haven Hospital and Winter Haven Women’s Hospital. .On October 6, the health system plans to go-live with the five remaining BayCare hospitals: Morton Plant Hospital in Clearwater, Mease Dunedin Hospital, Mease Countryside Hospital, Morton Plant North Bay Hospital and St. Anthony’s Hospital in St. Petersburg.

“This is just one of the many ways BayCare is trying to engage patients and provide them with useful tools to help them participate in their own healthcare,” said Dr. Greg Hindahl, vice president and chief medical information officer of BayCare Health System. “Seeing a physician or other medical provider is much more valuable if the patient has reminders about what medicines they should take, activities to avoid or who they should be following up with for further care if necessary.”

BayCare also connects patients to their clinical information through a patient portal, which re-launched last year as myBayCare.org. The portal lets patients throughout the health system see lab results immediately, find a doctor, ask questions, learn about upcoming health events and pay bills online.

“Patients want more access, transparency and control over their healthcare and decisions. And, consumers’ increased connectedness has changed the patient journey as well as their expectations and behaviors. For us, it’s about improving the patients’ and their families’ healthcare experience,” explained Raj Toleti, co-founder and CEO of HealthGrid, which developed CareNotify™. “HealthGrid and BayCare understand thisand have taken steps to transform patients from passive bystanders to active participants in their healthcare plans from admission to the moment they are discharged from the hospital and beyond.”

HealthGrid Partners with VITAS and Nicklaus Children’s Hospital to Provide Live, Two-Way Patient Data Communication

ORLANDO, FL – (April 13, 2015) – HealthGrid, a provider of solutions that vastly improve the communication of patient data throughout the care continuum, today announced it has completed the rollout of a landmark mobile patient data exchange solution with partners VITAS Healthcare and Nicklaus Children’s Hospital (formerly Miami Children’s Hospital). As the first live, two-way system of its kind, the solution has the potential to transform healthcare collaboration by virtually eliminating delays, errors and disruptions between upstream and downstream healthcare providers.

The HealthGrid/VITAS/NCH solution is mobile-based, meaning clinicians can exchange critical care data securely via any smartphone or tablet. The ONC-certified system, incorporating HealthCare’s CareNotify and CareNarrative products, satisfies many key requirements of the CMS Meaningful Use Phase 2 (MU2).

VITAS reports that the electronic solution has a zero transcription error rate; it has also shortened call times dealing with referrals from an average of 7 minutes to just 1.25 minutes—a reduction of more than 82%. VITAS also states the amount of relevant data for each patient has increased.

“When you look at all the initiatives—ACA, HITECH and others—to make interoperability work, this initiative shows that secure, practical, two-way interoperability between partners is within our grasp,” said Patrick Hale, Chief Information Officer and Senior Vice President of VITAS Healthcare, the nation’s largest provider of end-of-life care. “Best of all, our install, which took only 10 hours over a seven-day period, demonstrates that we can make it happen without breaking the bank.”

30% Reduction in Staff Time HealthGrid’s mobile collaboration products facilitate the transfer of summary of care records and other discrete patient information between care providers electronically. CareNarrative provides a view of a patient’s clinical state and allows providers to communicate in real time about patient care. CareNotify, the second HealthGrid product, sends patient schedules, medication lists, nutritional updates and other relevant educational content to patients and their families instantly, yet without extra work from attending clinicians.

VITAS states that HealthGrid’s mobile collaboration platform has created major benefits that improve efficiency and overall quality of care, both for its own facilities as well as centers like Nicklaus Children’s Hospital. VITAS, as integrated hospice and palliative services provider, operates 50 hospice programs in 16 states and the District of Columbia and employs nearly 300 care teams active across its system. It has nearly 14,000 lives under its care on any given day.

“By using automated interfaces like HealthGrid, our staffs are no longer forced to locate or track faxed information. Data is instantly associated with the patient and never lost,” Hale reported. “Once we have a referral source connected via HealthGrid, we estimate a 30% reduction in time spent by nurses and call centers to locate patient information. This is an improvement that directly frees up nurses to be at patient bedsides where we want them to be.”

“As all healthcare providers know, data risk is always a possibility during transitions in care,” added Ed Martinez, Senior Vice President and Chief Information Officer, Nicklaus Children’s Hospital, formerly Miami Children’s Hospital. “These risks can cause misunderstandings about patient treatments. The HealthGrid connection with VITAS ensures there are no disruptions in the speed or accuracy of information. Moreover, communications with clinicians, patients and families are made in a format they’re used to seeing. This is comprehensive value that until now has been very elusive for all of us in modern healthcare.”

MU2 Thresholds Exceeded
According to VITAS' CIO, certain requirements of Meaningful Use Phase 2 for electronic transmission of patient data can be met by the platform linking the two organizations' networks. "MU2 rules dictate that data for 10% of patients be electronically transmitted. This solution can be used to transmit data to all referral partners with little technical effort," Hale noted. "Our goal is to have all referrals come in electronically, from all our partners. As a critical care provider, we have to be able to trust the data we receive. This is a near-bulletproof process that allows us to make sure every patient is treated with the same level of care."

Operational Savings
Both executives at Nicklaus Children’s Hospital and VITAS agreed that implementation of the collaborative platform was far easier than most integration systems. "Our experience is that most integrations take on the order of two months," Hale said. "HealthGrid required just 10 hours of work over a seven-day period."

Because information is automatically updated in patient electronic medical records by HealthGrid, VITAS now uses just three people nationwide to handle data requests, resulting in staff savings of $390,000. “From acute care to hospice or home settings, we want all partners to be on a system like this,” Hale said. “This is what fixes healthcare. Our holy grail is interoperability—there’s not one of us in health IT who doesn’t get that. Having meaningful data in real time, enabling care givers to intervene in order to meet patient demands, is transformational.”

ONC Certified Mobile Collaboration Platform

April 03, 2015

Introducing HealthGrid: First ONC Certified Mobile Collaboration Platform for Connecting Care Teams and Patients.

ORLANDO, FL – (April 03, 2015) – Seeking to solve one of the most difficult challenges in healthcare IT, experienced IT entrepreneurs Raj and Chakri Toleti have founded HealthGrid, a new company and product that vastly improves the communication of patient data to clinicians and patients throughout the care continuum. As a fully-integrated, secure mobile collaboration platform, HealthGrid simultaneously satisfies many CMS Meaningful Use Phase 2 (MU2) requirements by securely transmitting summary of care information between providers; it also gives patients and families access to discharge instructions on any mobile device.

HealthGrid is an end-to-end solution that makes critical patient information accessible and actionable at the point of need. The ONC-certified product is comprised of two services, each of which delivers only the data necessary for the specified purpose:

CareNotify™: A device-agnostic mobile solution that updates patient families with real-time information about their loved one’s important clinical events. Patient schedules, medication lists, nutritional updates and relevant educational content are made available automatically, yet without disrupting existing clinician workflows. In conjunction, patients are given access to their care summaries, thus helping providers achieve Meaningful Use requirements.

CareNarrative™: Provides a snapshot view of a patient’s clinical state and allows providers to communicate with each other about patient care. CareNarrative effectively delivers patient summary of care records during referrals and/or transitions of care; moreover, it satisfies MU2 mandates while supporting the dynamic creation and management of hybrid, DIRECT and secure non-DIRECT accounts. The integrated task management, notification and note creation features in CareNarrative allow care teams to access critical information across the care continuum.

“HealthGrid changes the way care teams, patients and families exchange critical information,” said Chakri Toleti, HealthGrid’s CEO. “Mobile is a way of life for both consumers and healthcare professionals. Our platform allows all parties to communicate with each other securely in real time, while also sending out alerts, reminders and other messages to patients. It bridges the discrete elements in the care pathway and smoothes the often harsh transitions between providers and patients.”

Proven Leaders
HealthGrid’s founders are veteran healthcare IT entrepreneurs. Raj Toleti is founder, and CTO of Coordinated Care Solutions at PatientPoint™, a provider of health information and engagement programs for patients and physicians. Previously he founded Galvanon with Chakri Toleti, a company eventually acquired by NCR, and Cytura Corporation, a content management company acquired by Mobius. Raj Toleti also was a founding member of the Microsoft Consulting Services (Healthcare) practice. Chakri Toleti, Founder and CEO of HealthGrid, also serves as CEO of Mahathi Software Pvt. He previously served as president of both Galvanon and Cytura.

“HealthGrid promises to transform the process of information exchange between clinicians and patients. It meets all HIPAA security requirements, yet is highly intuitive and requires little to no training,” noted Raj Toleti. “HealthGrid not only facilitates information sharing at the clinical level, but also encourages participation by patients and families as collaborative care partners.”

Successful Deployments
HealthGrid has already gained significant traction in the healthcare industry via deployments at several major institutional networks. Nicklaus Children’s Hospital (formerly Miami Children’s Hospital), a development partner in HealthGrid, has deployed and is currently using the solution in roughly two-thirds of its departments, with plans to extend HealthGrid systemwide.

“For patients and their families, the ability to understand what needs to be done, and at what time, is absolutely critical to positive outcomes,” said M. Narendra Kini, MD, President and CEO of Miami Children's Health System. “Having a solution that proactively notifies family members on a timely basis, wherever they are, is the prime value proposition of CareNotify™. When combined with CareNarrative, HealthGrid’s services enable us to bring coordination of care up to par with other advances in medicine.”
