HealthGrid honored as recipient of the 2017 Microsoft Health Innovation Awards

ORLANDO, Fla., Feb. 21, 2017 HealthGrid today announced it has been named a recipient of Microsoft Corp.'s 2017 Health Innovation Awards.

The awards, which were announced at the 2017 HIMSS Annual Conference and Exhibition in Orlando, Florida, recognize health organizations and their technology solution partners for using Microsoft devices and services in innovative ways that help engage patients, empower care teams, optimize clinical and operational effectiveness, and transform the care continuum. The 2017 winners are transforming the industry by creating breakthrough solutions that empower health and life sciences organizations, while meeting global, local and industry-specific compliance and security standards.

HealthGrid, the nation's fastest growing patient engagement platform, has demonstrated compelling evidence in the value it creates for provider organizations, like its partner Family Physician Group (FPG), making the organization the clear winner in the category for "Engage Your Patients." HealthGrid's Patient Engagement Platform creates a cohesive, mobile patient experience by proactively initiating bit sized, contextual interactions with the patient at the appropriate time throughout their care journey. With HealthGrid, FPG sought to improve the overall quality of care, optimize access to their physician network, improve compliance to regulatory requirements, like Meaningful Use and MACRA, while supporting the shift to value- based care. Leveraging the HealthGrid platform, FPG has fully attested all providers for Meaningful Use, added two more appointments per provider per day leading to 40 percent more closures of clinical care gaps, and 60 percent of patients utilizing post-care instructions and educational materials which has improved care plan compliance. As a result, HealthGrid has helped FPG achieve a 9:1 ROI resulting in $50k more per physician annually.

"We are so grateful to our partners at Family Physicians Group and our collaboration with Microsoft. This award is only possible by working with thought leaders in the industry who are reshaping the conversation between patient and physician. We have always understood that patient engagement is paramount to driving quality care. And with that, our focus remains on creating a meaningful, interactive patient experience for our customers," said Raj Toleti, president and co-founder of HealthGrid.

From pre-care administrative operations, including gaps in care outreach, scheduling and reminders, electronic check-in to clinical interactions, including post-care delivery of care summaries, referral coordination, and care plan management, HealthGrid offers a consistent touchpoint to patients while driving significant return to the practice.

"The health industry is undergoing a seismic shift in which intelligent technologies are helping organizations, communities and individuals improve care by helping them better understand and share information," said Laura Wallace, vice president, Health & Life Sciences, Microsoft. "This year's Microsoft Health Innovation Award recipients are advancing the goals of improved patient engagement and care coordination through their pioneering use of Microsoft devices, platforms and cloud services."

Nominations were submitted by health providers, payers, pharmaceutical and life science organizations, and public and private health institutions across the world for applying Microsoft technology to create transformative and highly-effective innovations. An esteemed panel of industry experts selected this year's winners based on how their innovation represents a forward-thinking development or implementation of a solution that is delivering groundbreaking results and producing better health outcomes for more people. Recipients will be highlighted on the Microsoft website at http://www.microsoft.com/healthinnovationawards and on the Microsoft in Health blog at https://enterprise.microsoft.com/en-us/industries/health/?post_type=articles.

HealthGrid Integrates with Microsoft Dynamics 365 to Automate Patient Interactions During Care Journey

Next generation CRM platform improves patient acquisition, retention, care plan management

ORLANDO, Fla., Feb. 15, 2017 – HealthGrid, a mobile patient engagement leader, today announced the integration of its mobile patient engagement platform with Microsoft Dynamics 365. The integration to Dynamics 365 enables health systems and providers using HealthGrid's patient engagement technology to automate interactions during the patient journey. As a result, health systems and providers can connect with patients at anytime and anywhere, and on any device to deliver specific care communications.

As each interaction occurs, HealthGrid captures specific patient feedback and pushes that data back into Dynamics 365, thus helping health systems and providers uncover rich and actionable data on the patient experience to improve:

  • Patient acquisition and retention – Identify patient populations' care gaps and when to conduct outreach to patients for those required services. The technology also ensures that patients can move to the required downstream providers in accordance with care plans.
  • Post-care to post-acute care plan management – With Dynamics 365, users can identify and enroll patients into HealthGrid's care plan management module allowing for better patient monitoring, including care plan follow up with physicians, compliance to medication therapies, and patient assessments.
  • Patient Relationship Campaigns – Health systems and providers can now boost the effectiveness of their educational and disease management programs, more easily enroll patients into these programs, and deliver individually tailored content via HealthGrid's mobile patient engagement platform.

"The mobile-forward capabilities within Microsoft Dynamics 365 are a powerful value-add for our health system and provider partners," said Raj Toleti, President and Co-Founder of HealthGrid. "This data driven approach gives providers better insight into patient needs at any moment in the healthcare journey with a more personalized response back to them. Now, health systems and providers can make care coordination teams and call center staff more productive because they are able to focus on patients with adverse responses, or those with the most complex set of needs."

HealthGrid's patient engagement platform enables healthcare providers to engage with patients on their mobile device at any point in the care continuum. The solution provides automated gaps-in-care outreach, digital check-in, co-pay collection, and post-visit education and follow-up. The relationship between HealthGrid and Microsoft continues to grow, leveraging the strengths of each other's core capabilities to create a more robust, personal patient experience.

"We're pleased with how HealthGrid has embraced the Microsoft suite of technologies to drive its vision," said Laura Wallace, Vice President of U.S. Health and Life Sciences, Microsoft Corp. "Like other consumer-focused industries, healthcare must meet patients' needs at all touch points in the care experience. By implementing Microsoft Dynamics 365 and HealthGrid health systems gain real-time tools that deliver impactful experiences to patients on their mobile devices at unprecedented levels of engagement, resulting in better quality and increased patient satisfaction."

HealthGrid Chosen as Preferred Patient Engagement Vendor by Major Health Systems Health systems attracted to its expanding bundled & alternative payment models

Orlando, FL – August 10, 2016 – HealthGrid is poised to advance healthcare providers’ ability to increase value and return on investment (ROI) by strengthening their MACRA (Medicare Access and CHIP Reauthorization Act) and MIPS (Merit-based Incentive Payment System) readiness. The company is one of the few mobile-first patient engagement platforms to offer this functionality, increasing its appeal to both physician groups and hospitals.

By reaching patients on their personal mobile devices, HealthGrid’s platform allows healthcare providers to increase both patient engagement and ROI under MACRA’s new proposed legislation. More specifically, HealthGrid’s platform assists providers with:

  • Patient care plan management for Alternative Payment Models (APMs)
  • Compliance for patient-facing quality measures within programs such as Meaningful Use, PQRS (Physician Quality Reporting System), etc.

As the nation’s medical system continues to transform from volume to value, MACRA will shift reimbursement from fee-for-service to pay-for-performance under MIPS and APMs, placing greater emphasis on patient engagement and patient outcomes. Using HealthGrid, providers can improve care coordination for value-based care through a platform that combines patient mobile outreach, personalized education, mobile preventative screenings and assessments, care transition enablement, and care plan management.

HealthGrid’s CEO, Raj Toleti, stated that, “It is a must in the world of value-based payments to have the most effective electronic patient engagement strategy. At HealthGrid, our solutions provide mobile connectivity across the care continuum and help patients adhere to a variety of care plans pre and post care with the highest levels of patient engagement and care plan compliance in the country. As a result, we’ve been able to prove sustainable value through ROI-based use cases such as improved quality, reduced cost better care coordination, and increased revenue, while also aligning with regulatory requirements such as bundled and alternative payment models. This allows our clients to take a simple, consolidated patient-centered approach to care, while also preparing for the rigorous demands of an ever-changing reimbursement system.”

Healthwise and HealthGrid Announce Agreement to Offer Personalized Mobile Content for Deeper Patient Engagement

HealthGrid first in the nation to deliver personalized, intelligently-timed health information to patients on their mobile devices.

Orlando, FL – May 20, 2016 – Healthwise and HealthGrid unveiled a joint agreement today to deliver Healthwise's health education content to patients using HealthGrid's patient engagement solution. This collaboration will empower patients by combining customized patient education from the world's leading nonprofit provider with timely and intelligent technology that delivers information directly to patients before, during, and after health provider visits.

Healthwise and HealthGrid's partnership is expected to drive:

  • Higher patient engagement and satisfaction rates
  • More informed patient decision-making
  • Increased operational efficiency for health providers
  • Better patient outcomes

The partnership is on the forefront of a trend in healthcare towards encouraging patient engagement, education, and participation in patient's own health decisions. Patient engagement has been shown to increase patient satisfaction and reduce costs for healthcare providers, according to Dominick Frosch, PhD, a Robert Wood Johnson Foundation (RWJF) scholar and fellow at the Gordon and Betty Moore Foundation's Patient Care program. Recent studies show that patients are eager to access health information on their mobile devices: 72% of internet users say they looked online for health information within the past year, while 52% of smartphone owners have used their phone to look up health or medical information (Pew Research Center).

This collaboration will eliminate two major obstacles facing patient engagement: patient access and lack of physician time. Healthcare providers currently rely on paper print-outs during doctor visits and online portals that expect patients to know when and how to access the information they need. Using Healthwise's content and HealthGrid's CRM platform, curated, personalized content will be pushed to patients' phones via text message at the correct moment in their care journey.

"We are thrilled with this relationship. It will help to empower millions of patients around the world. By receiving high-quality health information on their own personal mobile devices, patients will be more educated, engaged, and have an increased ability to make better health decisions in partnership with their doctor and care team," stated Healthwise's President and COO Jim Giuffré.

"This combination of cutting-edge technology and highly respected, evidence-based content has the potential to revolutionize the patient experience," said Raj Toleti, HealthGrid's founder and CEO. "Both Healthwise and HealthGrid share a strong commitment to increasing patient engagement, and we are excited to partner in pursuit of this common goal."

BayCare Launches New Mobile Application for Patient Discharge Instructions

Clearwater, Fla., – To make it easier for hospital patients to transition back to their home environment, BayCare Health System is launching an application that allows people to receive discharge instructions on their mobile devices.

Utilizing the cell phone number provided during patient registration, this new application securely delivers and confirms receipt of electronic discharge instructions sopatients can see things like prescribed medications, diet, activity and recommended follow-up appointments. Meanwhile, BayCare providers can rest assured that this information can go anywhere their patients do.

MU2 Thresholds Exceeded
The innovative application, powered by CareNotify™, has been available for several weeks at St. Joseph's Hospital, St. Joseph's Children's Hospital, and St. Joseph's Women's Hospital in Tampa. Early indications show that more than 25% of patients are taking advantage of the new mobile technology.

Operational Savings
Today, the application was introduced tofive additional BayCare hospitals: St. Joseph's Hospital-South in Riverview, St. Joseph's Hospital-North in Lutz, South Florida Baptist Hospital in Plant City, Winter Haven Hospital and Winter Haven Women's Hospital. .On October 6, the health system plans to go-live with the five remaining BayCare hospitals: Morton Plant Hospital in Clearwater, Mease Dunedin Hospital, Mease Countryside Hospital, Morton Plant North Bay Hospital and St. Anthony's Hospital in St. Petersburg.

"This is just one of the many ways BayCare is trying to engage patients and provide them with useful tools to help them participate in their own healthcare," said Dr. Greg Hindahl, vice president and chief medical information officer of BayCare Health System. "Seeing a physician or other medical provider is much more valuable if the patient has reminders about what medicines they should take, activities to avoid or who they should be following up with for further care if necessary."

BayCare also connects patients to their clinical information through a patient portal, which re-launched last year as myBayCare.org. The portal lets patients throughout the health system see lab results immediately, find a doctor, ask questions, learn about upcoming health events and pay bills online.

"Patients want more access, transparency and control over their healthcare and decisions. And, consumers' increased connectedness has changed the patient journey as well as their expectations and behaviors. For us, it's about improving the patients' and their families' healthcare experience," explained Raj Toleti, co-founder and CEO of HealthGrid, which developed CareNotify™. HealthGrid Partners with VITAS and Nicklaus Children's Hospital to Provide Live, Two-Way Patient Data Communication

ORLANDO, FL – (April 13, 2015) – HealthGrid, a provider of solutions that vastly improve the communication of patient data throughout the care continuum, today announced it has completed the rollout of a landmark mobile patient data exchange solution with partners VITAS Healthcare and Nicklaus Children's Hospital (formerly Miami Children's Hospital). As the first live, two-way system of its kind, the solution has the potential to transform healthcare collaboration by virtually eliminating delays, errors and disruptions between upstream and downstream healthcare providers.

The HealthGrid/VITAS/NCH solution is mobile-based, meaning clinicians can exchange critical care data securely via any smartphone or tablet. The ONC-certified system, incorporating HealthCare's CareNotify and CareNarrative products, satisfies many key requirements of the CMS Meaningful Use Phase 2 (MU2).

VITAS reports that the electronic solution has a zero transcription error rate; it has also shortened call times dealing with referrals from an average of 7 minutes to just 1.25 minutes—a reduction of more than 82%. VITAS also states the amount of relevant data for each patient has increased.

"When you look at all the initiatives—ACA, HITECH and others—to make interoperability work, this initiative shows that secure, practical, two-way interoperability between partners is within our grasp," said Patrick Hale, Chief Information Officer and Senior Vice President of VITAS Healthcare, the nation's largest provider of end-of-life care. "Best of all, our install, which took only 10 hours over a seven-day period, demonstrates that we can make it happen without breaking the bank."

30% Reduction in Staff Time HealthGrid's mobile collaboration products facilitate the transfer of summary of care records and other discrete patient information between care providers electronically. CareNarrative provides a view of a patient's clinical state and allows providers to communicate in real time about patient care. CareNotify, the second HealthGrid product, sends patient schedules, medication lists, nutritional updates and other relevant educational content to patients and their families instantly, yet without extra work from attending clinicians.

VITAS states that HealthGrid's mobile collaboration platform has created major benefits that improve efficiency and overall quality of care, both for its own facilities as well as centers like Nicklaus Children's Hospital. VITAS, as integrated hospice and palliative services provider, operates 50 hospice programs in 16 states and the District of Columbia and employs nearly 300 care teams active across its system. It has nearly 14,000 lives under its care on any given day.

"By using automated interfaces like HealthGrid, our staffs are no longer forced to locate or track faxed information. Data is instantly associated with the patient and never lost," Hale reported. "Once we have a referral source connected via HealthGrid, we estimate a 30% reduction in time spent by nurses and call centers to locate patient information. This is an improvement that directly frees up nurses to be at patient bedsides where we want them to be."

"As all healthcare providers know, data risk is always a possibility during transitions in care," added Ed Martinez, Senior Vice President and Chief Information Officer, Nicklaus Children's Hospital, formerly Miami Children's Hospital. "These risks can cause misunderstandings about patient treatments. The HealthGrid connection with VITAS ensures there are no disruptions in the speed or accuracy of information. Moreover, communications with clinicians, patients and families are made in a format they're used to seeing. This is comprehensive value that until now has been very elusive for all of us in modern healthcare."

MU2 Thresholds Exceeded
According to VITAS' CIO, certain requirements of Meaningful Use Phase 2 for electronic transmission of patient data can be met by the platform linking the two organizations' networks. "MU2 rules dictate that data for 10% of patients be electronically transmitted. This solution can be used to transmit data to all referral partners with little technical effort," Hale noted. "Our goal is to have all referrals come in electronically, from all our partners. As a critical care provider, we have to be able to trust the data we receive. This is a near-bulletproof process that allows us to make sure every patient is treated with the same level of care."

Operational Savings
Both executives at Nicklaus Children's Hospital and VITAS agreed that implementation of the collaborative platform was far easier than most integration systems. "Our experience is that most integrations take on the order of two months," Hale said. "HealthGrid required just 10 hours of work over a seven-day period."

Because information is automatically updated in patient electronic medical records by HealthGrid, VITAS now uses just three people nationwide to handle data requests, resulting in staff savings of $390,000. "From acute care to hospice or home settings, we want all partners to be on a system like this," Hale said. "This is what fixes healthcare. Our holy grail is interoperability—there's not one of us in health IT who doesn't get that. Having meaningful data in real time, enabling care givers to intervene in order to meet patient demands, is transformational."

ONC Certified Mobile Collaboration Platform

April 03, 2015

Introducing HealthGrid: First ONC Certified Mobile Collaboration Platform for Connecting Care Teams and Patients.

ORLANDO, FL – (April 03, 2015) – Seeking to solve one of the most difficult challenges in healthcare IT, experienced IT entrepreneurs Raj and Chakri Toleti have founded HealthGrid, a new company and product that vastly improves the communication of patient data to clinicians and patients throughout the care continuum. As a fully-integrated, secure mobile collaboration platform, HealthGrid simultaneously satisfies many CMS Meaningful Use Phase 2 (MU2) requirements by securely transmitting summary of care information between providers; it also gives patients and families access to discharge instructions on any mobile device.

HealthGrid is an end-to-end solution that makes critical patient information accessible and actionable at the point of need. The ONC-certified product is comprised of two services, each of which delivers only the data necessary for the specified purpose:

CareNotify™: A device-agnostic mobile solution that updates patient families with real-time information about their loved one's important clinical events. Patient schedules, medication lists, nutritional updates and relevant educational content are made available automatically, yet without disrupting existing clinician workflows. In conjunction, patients are given access to their care summaries, thus helping providers achieve Meaningful Use requirements.

CareNarrative™: Provides a snapshot view of a patient's clinical state and allows providers to communicate with each other about patient care. CareNarrative effectively delivers patient summary of care records during referrals and/or transitions of care; moreover, it satisfies MU2 mandates while supporting the dynamic creation and management of hybrid, DIRECT and secure non-DIRECT accounts. The integrated task management, notification and note creation features in CareNarrative allow care teams to access critical information across the care continuum.

"HealthGrid changes the way care teams, patients and families exchange critical information," said Chakri Toleti, HealthGrid's CEO. "Mobile is a way of life for both consumers and healthcare professionals. Our platform allows all parties to communicate with each other securely in real time, while also sending out alerts, reminders and other messages to patients. It bridges the discrete elements in the care pathway and smoothes the often harsh transitions between providers and patients."

Proven Leaders
HealthGrid's founders are veteran healthcare IT entrepreneurs. Raj Toleti is founder, and CTO of Coordinated Care Solutions at PatientPoint™, a provider of health information and engagement programs for patients and physicians. Previously he founded Galvanon with Chakri Toleti, a company eventually acquired by NCR, and Cytura Corporation, a content management company acquired by Mobius. Raj Toleti also was a founding member of the Microsoft Consulting Services (Healthcare) practice. Chakri Toleti, Founder and CEO of HealthGrid, also serves as CEO of Mahathi Software Pvt. He previously served as president of both Galvanon and Cytura.

"HealthGrid promises to transform the process of information exchange between clinicians and patients. It meets all HIPAA security requirements, yet is highly intuitive and requires little to no training," noted Raj Toleti. "HealthGrid not only facilitates information sharing at the clinical level, but also encourages participation by patients and families as collaborative care partners."

Successful Deployments
HealthGrid has already gained significant traction in the healthcare industry via deployments at several major institutional networks. Nicklaus Children's Hospital (formerly Miami Children's Hospital), a development partner in HealthGrid, has deployed and is currently using the solution in roughly two-thirds of its departments, with plans to extend HealthGrid systemwide.

"For patients and their families, the ability to understand what needs to be done, and at what time, is absolutely critical to positive outcomes," said M. Narendra Kini, MD, President and CEO of Miami Children's Health System. "Having a solution that proactively notifies family members on a timely basis, wherever they are, is the prime value proposition of CareNotify™. When combined with CareNarrative, HealthGrid's services enable us to bring coordination of care up to par with other advances in medicine."
